Today I want to talk about the use of cosmetic fillers and threads, share my experience and observations.

Fillers are another cosmetological invention, which, along with Botox, has made it possible to significantly change the appearance of women 35+. Unfortunately, not always for the better, but I refuse to see only the pillow faces of women who are not familiar with moderation as an example. To the point of absurdity, especially if you have the time and resources, you can take anything you want - from a healthy lifestyle to teeth whitening, but this does not make either one or the other a phenomenon excessive and disfiguring in its essence.

Fillers are a unique tool that helps both prevent and correct many problems caused by aging, mainly united by the short but capacious word “ptosis”. Essentially, these are dense gels of varying densities based on biodegradable (that is, completely excreted from the body) hyaluronic acid.

Prevention, as I have said many times, is easier. Fillers allow you to create a frame that will “hold your face” longer - oval, nasolabial, sunken eyes.

We don’t even always understand that our “tired” face, which settled in the mirror after 30, no longer goes away with the onset of more favorable factors such as sufficient sleep, vacation, regaining weight lost as a result of stress, etc. – this is an age-related manifestation and is not always 100% reversible.

It is caused by the loss of adipose tissue, which will only continue to worsen and eventually turn into deep nasolacrimal grooves, jowls, pronounced nasolabial folds and other signs of ptosis.

To a greater extent and much earlier, owners of oily and combination, dense skin face this. For those who have dry skin, a deficiency of adipose tissue is much less likely and will not be very noticeable - because there is little of it to begin with.

Let me remind you of my initial data. I am 40, I have thick and heavy skin, very, no VERY prone to ptosis. I have been fighting to maintain the oval and volume in the right areas since I was 30 years old. Before my eyes, by the age of 36, my mother had acquired such bulldog cheeks, jowls and other gravitational “charms” that I had no illusions about this.

Therefore, fillers appeared in my life quite early - the first time I corrected nasolabial folds was at 32 years old. But it didn’t work the first time.

The fact is that understanding where to inject fillers in order to get an effect specifically on one or another problem area is the essence of a cosmetologist’s professionalism, which, as I then discovered, not everyone has. The cosmetologist with whom I first consulted about my nasolabial lips suggested injecting filler into my nasolabial lips.

It would seem that everything is logical, but no. I don’t know where this came to me then, I had zero experience in injections, but for some reason I immediately realized that injecting fillers into the formed fold was illogical and pointless. You need to prick where there is a lack of volume and where the excess skin has “slipped off”. In my case it was the upper parts, “apples”, cheekbones.

Nasolabial folds are still actively corrected by introducing filler into the fold itself; the gel seems to push it out from the inside, smoothing the skin. I still find it very ineffective, but everyone's face is different and maybe it works for some. I have seen the result on others: on young skin and with mildly expressed folds, the first 1-2 times it looks tolerable, but I still see that instead of a fold on the face there is now a roller that is not noticeable from the front, but is visible from other angles . With age, this method stops working altogether.

I left that cosmetologist and saw my next doctor for the next 4.5 years. So she just suggested injecting the gel into the cheekbone area to “raise” the nasolabial lips.

In my case, the injection was at the point marked in the figure above. It worked like a charm. The seemingly small volume added to the cheekbones smoothed out the areas between the cheekbones and the corners of the mouth, without changing my face “beyond recognition.” From the point of view of physiognomy, nothing has changed - neither swollen eyes nor swollen cheeks appeared. We are talking about a small point volume, that is, again, about moderation. Over time, when by the end of the filler’s validity period not only nasolabial folds appeared, but also signs of sagging of the oval, I began to notice that after the same procedure, the oval also fell into place. This is exclusively the professionalism of a cosmetologist who understood what vector of gravity my muscles and tissues have and how to work with it. We did not change either the volume or density of the gel during this time. But the effect was excellent every time.

As for rehabilitation, it should be noted that I am not prone to severe swelling, and the period when at least some consequences of the intervention were visible (in my case it was swelling in the areas of injection and location of the filler) lasted Saturday and half of Sunday , if I did the procedure on Friday after work.

The effect of such injections lasts from 1 to 2 years, but we remember that everything is individual. For me it was closer to a year than two, but I waited 1.5 years between procedures. After 35, I added filler of a lower density to the nasolacrimal groove, which by this age was very clearly visible, making the overall appearance tired, shadows lay under the eyes, although this was more of an optical effect from the lack of volume immediately above the orbital bone.

It must be said that fillers are also used to correct existing wrinkles, for example on the forehead and under the eyes. I didn’t have to do this, because I regularly inject Botox into my forehead, and there are no wrinkles there, but in the area around my eyes I have very active facial expressions and the filler there, I suspect, will last poorly and not for long.

Threads

During these procedures, for almost 5 years, my face hardly changed, in the good sense of the word. But closer to 37, I decided on threads. Somehow, by accident, due to my profession, my close circle was denied a source of reliable information almost first-hand. I asked everyone, looked at real examples of the work of the doctor who invented and patented the iconic and to this day most used X threads, the name of which, apparently, it’s better not to mention here just in case, so that it doesn’t look like an advertisement. And I decided to use threads first in the middle third of the face. This is the top bunch of red lines in the picture:

In my understanding, threads perfectly solve both short-term problems - they well correct existing (in my case not very pronounced) ptosis, and long-term ones - they reinforce problem areas and keep them from sagging for several years.

X threads with polylactic acid are made of a material similar to surgical threads for intracavitary sutures, which dissolve over time and are removed from the body. These threads have “notches” that allow them to better “cling” to the internal tissues, fix the threads well, allow the skin to be evenly stretched, and avoid the migration of threads under the skin and other horrors. The polylactic acid in the composition helps to form a collagen framework in place of the threads, which continues to “hold” the affected area for some time after their resorption.

The threads are introduced in a fan through one point, the coverage area can be quite large.

The effect of the threads can last from 2 to 5 years, depending on the density of the threads and the characteristics of the skin. I chose threads for myself as an alternative to fillers with a longer duration and better results, including for the future.

Yes, threads will not compensate for the deficiency of adipose tissue, and will not solve the problem of the nasolacrimal groove, for example. Further filler injections may be needed there. But they do mechanical lifting, correction of nasolabial, oval, jowls of varying degrees of severity very cool.

Threads are undoubtedly more expensive, not all even very advanced cosmetologists know how to make them (at least very well), and the rehabilitation after them is much more serious. But in my case, again, we are talking about a slightly more noticeable and slightly longer-lasting discomfort; I am not inclined to painful rehabilitation, in principle, on all medical fronts. There are people who are almost ready to take sick leave after a thread lift, they are so uncomfortable and, probably, even in pain.

In my case, the result was definitely worth it. I expected a miracle, but the effect exceeded even my wildest expectations. The next day (!) after the procedure, I saw a friend whom I had not seen for more than a year and who, due to her profession, also had a keen eye for various interventions. She didn't suspect anything. And only three times during lunch I repeated that I looked very good.

I did the second procedure three years later and, in addition to the middle third, I also put threads on the lower third, because at the threshold of 40, the ptosis was getting worse and worse under persuasion. I'm happy with the result again. This time I used denser threads, so I hope that a repeat will be needed in more than three years.

The threads on my face are invisible. They are not noticeable to anyone who turns to normal specialists, does not skimp on the threads themselves and is friendly with their head. They are not noticeable either in facial expressions or when losing or gaining weight (which is not very typical for me, but +/- 3 kg from winter to summer happens), they do not interfere, they are not felt, they simply are not there.

We all accept (or not accept) our age-related changes in different ways. And you can consider this my point, but it is important for me to preserve the oval and well-fitted skin on my face (it doesn’t sound very good, but I want to convey the idea). I have well-defined wrinkles under and around my eyes from very active facial expressions, nothing takes away from them, and this is the only way they show age. But they don't upset me. I care for them and cherish them, I don’t regret creams, serums, patches, but I don’t have high hopes and they don’t interfere with my life. I hardly look 25 or even 35 at 40, and there is no such task. But ptosis of the face disfigures it, makes them dull ahead of time, and I’m glad that now there are such prevention options, I don’t regret a single procedure done.

For the sake of breadth of knowledge, I can also add that, in addition to absorbable threads, there are also non-absorbable threads. I believe they come into use at a more mature age with more severe ptosis.

There are also so-called mesothreads. This is a rather vague category. They came from Korea about 8-10 years ago, as a cheaper alternative to X threads and their analogues, which at that time were at the peak of their rise, but both then and now had a serious drawback - high cost. “Serious” thread cosmetologists call mesothreads sub-threads and do not take them seriously. For me, I think they are a little weak, but since they are still on the market, perhaps they have found their niche. Their difference from ordinary threads is that they are thinner and less traumatic. And they cannot do mechanical lifting comparable to X threads. I have no experience using them, maybe someone will share in the comments.

Neither I nor most of my friends have problems with a sense of proportion, so I find this only a benefit for the modern woman. No amount of care will protect against natural aging, but it can delay and soften the manifestation of age-related signs. I am deeply convinced of this.
