History of thread lifting with mesothreads

This technique for correcting age-related changes in various parts of the body is the result of the developments of specialists from Korea. They used all the strengths of modern technology and traditional acupuncture methods.

It is known that our body contains 14 main energy channels, which are also called meridians. They travel throughout the body, connecting organ systems, limbs, face and torso into one large structure. Unfortunately, rejuvenation through surgery is not suitable for every woman. In addition to the contraindications, many are put off by the need for long rehabilitation.   • in our own practice (since 2012), the Aptos thread lifting technique occupies a small share, which is primarily due to the emergence of a worthy, high-quality alternative - a new technology of non-invasive tissue tightening - ultrasonic SMAS-lifting, which allows achieving high-quality clinical results without a rehabilitation period with a stable clinical result;
  • As for the second aspect of “mesothreads” - improving the structure of the skin, this raises the question of the feasibility of achieving the effect of “reinforcing” the skin for 3-4 months (the period of thread resorption) with such trauma and potential risks; Regarding the revitalizing effect due to the introduction of a synthetic inert material into the thickness of the skin, I, as a dermatologist, have a question - due to what? And what did traditional, time-tested hyaluronic acid preparations, mesotherapy, and plasma lifting not like in this matter?

3. Today there is a stable information field:

  • on the one hand: there is a wary or negative reaction from European colleagues - there is a large flow of patients with complications after mesothreads;
  • on the other hand: there is active promotion on the part of mesothread manufacturing companies and trainers engaged by them, and at the same time there is a lack of adequate explanations about complications from these same trainers.

4. The subject for discussion is the effectiveness and safety of mesothreads for patients. There are no questions about their cost-effectiveness for doctors.

About mesothreads

The threads that specialists use during thread lifting are most often produced in South Korea. They are divided into the following types or generations:

  1. The first generation of threads are single-chain polydioxane threads made from synthetics; they usually disintegrate on their own and are excreted from the body.
  2. The second generation of threads differs from the previous type in increased strength and a special external coating.
  3. Third generation mesothreads consist of substances similar to previous varieties. The difference is only in the specific structure.

The threads used do not cause an allergic reaction. Cases of allergies are extremely rare and are associated with the specifics of the client’s body. In addition, the thread is non-toxic and is used in surgery. In other words, there is practically no reaction from the skin during resorption. Once in the desired area, the thread breaks down into water and carbon dioxide. It takes about eight months for the thread to decompose, after which a small compaction is retained and used as a frame.

Threads for 3D lifts can be different. There are several materials that differ in thickness, structure, and length. Specialists can use one type or several - then different parts of the body or face are processed. The specific type of thread is determined by the doctor, who takes into account the severity of age-related changes and the characteristics of the skin.

Linear mesothreads. Made from smooth and thin fibers, they are used to correct the shape of the lips, and correct problems localized around the eyes, upper eyelids, and neck. The length ranges from 25 to 90 millimeters.

Knowing the manufacturers is very important, because in cosmetology you may be asked to carry out the procedure using other threads. And in this case, there are no guarantees that the material will be of high quality, safe and will take root well. What is the method?

Its essence consists of subcutaneous injection into different parts of the body of the thinnest, almost invisible threads of polydioxanone.

A thread attached to a flexible steel guide needle, sharpened by a laser, is inserted in the form of a mesh perpendicular to the muscle tissue. The needle is so thin that it easily slides into the subcutaneous fat tissue, practically without damaging it. At the moment the needle moves back, the thread loses connection with the conductor and remains in the skin. The material is biologically compatible and absolutely safe for the human body. A very important point in the prevention of infectious complications:

  • thorough make-up removal of the entire face;
  • double treatment of the skin, including the hairline, with an antiseptic solution;
  • insulation of hair and neck with secured sterile napkins;
  • after implantation of the threads, the skin is carefully treated with an antiseptic;
  • Before the procedure, quartzing is carried out in the office.

Typical adverse events after the procedure are local hemorrhages and swelling. At the stage of mastering the technique, one of my patients developed an extensive hematoma in the temporal region, which “slipped” onto the cheekbone. The patient complained of pain during articulation and chewing. Another patient developed severe swelling that persisted for more than a week. Regarding other undesirable phenomena, in my practice there were only two cases: displacement and cutting of the thread 3 days after the procedure. We removed the “erupted” thread, but we could not remove the displaced thread; we are monitoring the patient. A patient of one of my colleagues had an infectious complication - an abscess of the soft tissues of the cheek. The abscess was treated surgically against the background of antibacterial therapy, anti-inflammatory and desensitizing therapy. Also, once I consulted a patient with visualization of threads under the skin when they were implanted too superficially (they were visible with blue stripes). The threads have been removed.

In my experience, the most common adverse event is lack of results!

To summarize, I would like to say that the method and materials are promising, but require additional study. Of course, we want the promises that companies make to us and that we convey to patients to come true.

As a practicing doctor, I would like to draw attention to a number of facts and share my own opinion about the “revolutionary technique” - mesothreads:

  • the technique was developed and began to be used in South Korea several years ago [6];
  • leading specialists in the field of aesthetic medicine have little experience in using this technique;
  • in Ukraine, the widespread use of the technique began even later than in Russia, therefore, there is no talk about the presence of significant practical experience and, as a result, an understanding of the effectiveness (expediency) of the technique, the risks of developing immediate and long-term consequences;
  • At the time of writing this article, “mesothreads” are not widely used both in Western Europe and in the USA; the technique is not certified by the FDA (USA), which is a “rule of good manners” for all cosmetic products, equipment and techniques, and medical products.

Every doctor in his practice constantly faces certain “result/safety” when deciding on the advisability of a particular treatment method, a particular procedure. What do we have in “mesothreads”?

Safety

For the vast majority of patients, the rehabilitation period takes about a week. The most common adverse events observed after the introduction of polydioxanone threads are hemorrhages, pain and swelling of the implantation area.

In just 6 months of 2013, we observed more than 20 patients who underwent the procedure of placing “mesothreads” in various institutions in Donetsk and the region with more serious complications:

  • purulent inflammation along the threads (violation of the rules of asepsis and antisepsis when performing the procedure);
  • aseptic inflammation that developed several weeks after the insertion of the threads (probably, we are talking about an allergic reaction of the body to the inserted suture material) is the most complex complication, which is extremely difficult to correct even surgically;
  • contouring/protrusion of the ends of the threads under the skin (violation of the technique for placing the threads, or nuances of the technique itself).

Moreover, these are only those cases when the doctor who performed the procedure was unable to solve the problem that arose and the patient went to another specialized clinic, but how many such cases are there in total?

Efficiency

Manufacturing companies announce two effects achieved when installing “mesothreads” – lifting (skin tightening and improvement of facial features) and improving the quality characteristics of the skin [6].

  • in modern aesthetic medicine, there are at least two directions aimed at achieving a lifting effect (threads with protrusions or cones are more effective in this regard - Aptos or Silhouette Lift; hardware technologies operating at different levels - CO2 lasers, radio wave lifting, ultrasonic SMAS- lifting), which have a more pronounced effect and, what is important, a clear, high safety profile, confirmed by both European certificates and the FDA (USA);
  • improving the quality characteristics of the skin: in order to “reinforce” the skin, technologies such as intradermal and subcutaneous injections of stabilized hyaluronic acid, calcium hydroxyapatite preparations (in order to moisturize and improve metabolic processes in the dermis - injections of unstabilized hyaluronic acid (biorevitalization) and mesotherapy have been successfully used for a long time )), in order to improve microcirculation, the functioning of fibroblasts and the quality of metabolic processes of the skin - injections of PRP (platelet-rich plasma - plasmolifting). And this is not counting the whole arsenal of various methods of hardware cosmetology. Isn't this enough?! The main thing is that each of them has a result that is understandable to the doctor, achievable, and a confirmed safety profile.

What makes a doctor choose “mesothreads” given such a “losing balance: effectiveness/safety”?

  • on the one hand, this is the desire for something new, more perfect, effective (but you need to be objective and differentiate the promises of manufacturing companies from the results of clinical studies, documented confirmation of effectiveness and safety by quality committees in Europe and the USA);
  • on the other hand, I’ll just express my guess - the cost of one thread is $2.5-7.5 (depending on the manufacturing company), 30-40 threads are usually used to treat the face, the average cost of the procedure is $940. I think economics is interesting.

Indications and contraindications for the procedure

Almost any woman can make herself young and beautiful with the help of a thread lifting procedure. It is worth noting that any cosmetic effect has its own indications and contraindications - including mesothreads.

First, let's talk about the indications for this procedure:

  • folds in the nasolabial triangle;
  • wrinkles localized in different places;
  • drooping eyelids or eyebrows;
  • double chin;
  • loose skin in various parts of the body.

How do you prepare for thread lifting and how is it carried out?
